小さな手術も、大きな手術も、麻酔から覚醒に至るまでの間は常に細心の注意をはらいます。犬と猫における鎮静や麻酔に伴う死亡率は0.15-0.3%と報告されています。周術期（手術前、手術中、手術後）の安全を確保するため、全ての手術で麻酔を必要としますが、麻酔の安全性に影響するような基礎疾患が隠れていないか、麻酔前に血液検査やレントゲン検査などで確認をします。安全性の確保はもちろん、苦痛を軽減するべく周術期には疼痛管理（痛み止め処置）も行います。

We take every precaution to make sure your pet comes out of any procedure, whether major or minor, on the way to a swift recovery. A scientific study showed the death rate under sedation or anesthesia is around 0.15-0.3% on average.
To ensure your pet’s safety, we perform all pet surgeries under anesthesia and advocate the use of pre-anesthetic blood tests and X-ray examinations to detect any underlying disease that may affect the response to anesthesia.
With your pet’s comfort and safety at the forefront, we utilize pain management protocols before, during, and after surgery until they are completely recovered.

通常、麻酔前検査として血液検査とレントゲン検査を行います。血液検査では血球数、臓器の機能（肝機能や腎機能など）、血液凝固能（正しく血が止まるか）を評価し、胸部レントゲン検査では心臓や肺の状態を確認します。これらの検査により、手術が安全に実施可能か、また術後に問題が生じないかを判断することが可能です。必要に応じて腹部レントゲン、超音波検査、心電図検査などを追加します。

We usually perform blood tests and x-ray examinations prior to surgery. The blood tests can evaluate the blood counts, organ functions, and clotting functions. The chest x-ray can evaluate the conditions of heart and lung of your pet. The pre-anesthetic blood screening and x-ray are done to assure safety during surgery and the ability to heal following surgery. Additional examinations, such as abdominal x-ray examination, ultrasonography, electrocardiograms and other tests will be performed if required.

近年の獣医療の発展に伴い、以前と比べて麻酔と手術を安全に実施できるようになりました。麻酔前検査で体調をしっかりと評価し、状態に合わせて推奨される麻酔薬を選択します。一見健康そうに見えても、血液検査やレントゲン検査で深刻な問題がみつかることがあります。術前検査で異常が確認された場合は、状態が改善するまで手術を延期することがあります。

誤嚥を防ぐために、麻酔前は絶食が必要です。
・食餌：8-10時間の絶食が必要です（当日は何も食べさせない）。
・飲水：来院するまでは飲んでかまいません。
※真夏の間や、基礎疾患を有する場合は飲水制限をしない場合があります。

Today's modern anesthetic monitors have made surgery much safer than in the past. We do a thorough physical exam on your pet before administering anesthetics. We also adjust the amount and type of anesthetic used depending on the health of your pet. Even apparently healthy animals can have serious organ system problems that cannot be detected without blood testing or X-ray examination. If serious problems are detected, surgery can be postponed until the problem is corrected.
It is important that surgery be done on an empty stomach to reduce the risk of vomiting during and after anesthesia. You will need to withhold food for at least 8 to 10 hours before surgery. Water can be left down for the pet until the morning of surgery. *Fresh water should be available in mid-summer or if your pets have underlying disease.

ほとんどの手術において、体の中には吸収糸（溶ける糸）を使います。自然と溶けるので抜糸の必要がありませんが、完全に吸収されるまでには180-240日以上を要します。時々、溶ける糸が縫い目から見える場合がありますが、心配はいりません。
通常当院では皮内縫合を行うことが多いため、抜糸を必要としません。大きな手術や特殊な手術(腫瘍外科や整形外科など)では皮膚を非吸収糸で縫うことがあり、抜糸を必要とすることがあります。
いずれの場合も術後は、ご自宅にて術創の腫れや漿液の排出がないか注意してご観察頂く必要があります。わんちゃんやねこちゃんは自分で糸を噛み切ってしまったり、術創を舐めたりすることがありますので、十分にご注意下さい。抜糸まではお散歩やお風呂を控える必要があります。

For many surgeries, we use absorbable sutures underneath the skin. These will dissolve on their own, and do not need to be removed later. It takes 180-240days or more to complete absorption. In surgeries with smaller incisions, we usually perform subcuticular sutures which do not require suture removal. Some surgeries, especially tumor removals or orthopedic surgery, do require skin sutures. With either type of closure, you will need to keep an eye on the incision for swelling or discharge. Most dogs and cats lick excessively or chew at the incision, therefore this is an occasional problem you will also need to watch for. You will also need to limit your pet's activity level for a time and no baths are allowed until suture removal.

通常は術後10-14日で抜糸が可能です。術創の治り具合や、切開創の大きさにより多少前後します。

Non-absorbent sutures are usually removed 10-14 days following the surgery. Suture removal is determined by how well the wound has healed and extent of the surgery.

人と同様に動物も痛みの管理が必要です。人と異なる点として、明確に痛みを訴えずに我慢をしていることがしばしばありますが、痛みがあることは確かです。必要な痛み止めは手術部位や侵襲の程度によって異なります。

周術期の疼痛管理として複数の鎮痛薬を併用します。それでも痛そうな場合は、鎮痛薬を追加投与します。患部の違和感や腫脹が顕著な場合には、退院後も数日間は消炎鎮痛薬を内服して頂きます。

人間用の痛み止めには動物に有害な物があります。ロキソニンなどの消炎鎮痛薬は犬や猫に重篤な消化管潰瘍を発生させる危険性がありますので、絶対に投与しないでください。また、獣医師の指示がない限り、市販の薬剤の投与はお控えください。

Anything that causes pain in people can be expected to cause pain in animals. Pets may not show the same symptoms of pain as people do; they usually don't whine or cry, but you can be sure they feel it. Pain medications needed will depend on what surgery performed. Major procedures require more pain relief than things like minor lacerations.

We administer several analgesic agent as multi-modal analgesia for perioperative pain management. Any animal that appears painful will receive additional pain medication.
After surgery, we may recommend an oral anti-inflammatory drug the day after surgery and several days if discomfort or swelling are apparent.

Almost pain relief drugs for human may harmful for dogs and cats. For example, Nsaids for human, such as LOXONIN (loxoprofen sodium hydrate) administration may cause intestinal ulceration in dogs and cat. Therefore, please do not give your household medications to your pets, unless directed by veterinarian.

麻酔中に実施できる他の処置があります。外耳炎処置、マイクロチップ挿入や爪切りなどです。病院にお連れになる方が、主たる飼い主様でない場合はあらかじめご家族でよくご相談頂く必要があります。

術前には検査とご説明に30-60分間ほどを必要とします。退院時も10分間以上のご案内をさせていただきます。

While your pet is under anesthesia, it is the ideal time to perform other minor procedures, such as nail cut, ear cleaning, or implanting an identification microchip. If you would like an estimate for these extra services, please call ahead of time. This is especially important if the person dropping the pet off for surgery is not the primary decision maker for the pet's care.

When you bring your pet in for surgery, we will need 30 to 60 minutes of time to fill out paperwork and make decisions on the blood testing and other options available. When you pick up your pet after surgery you can also plan to spend about 10 minutes or more to go over your pet's home care needs.
